Is this gynecomastia: My nipples are puffy and point outwards. I am 5'7 and weigh 155 lbs. (Photos)

The upper part of my chest does not have much fat but the bottom does i don’t know if its fat or if its Gynecomastia. 

Is it a different kind of gynecomastia if you don't have the puffy

I currently have gyno, and I want to get surgery. However, I am afraid that estrogen within my body will reverse it.
